Because hotels and restaurants that are patronized during the Wheat State Whirlwind Tour prohibit smoking, and to ensure fairness in our accomodations for tour members, we request that you answer the following question truthfully. Do you smoke?



Do special circumstances require that you have a private room? (If you are selected, you will be asked to submit a check for $250 made payable to the University of Kansas. Please do not send payment until after notification that you have been accepted.)
